NORTH SHORE HOSPICE & PALLIATIVE PROJECT
We work in coordination with the North Shore Palliative Program, an integrated program for seriously ill patients that is operated by Vancouver Coastal Health Authority. Patients are referred to the program by their doctors or home health care professional and each patient is assessed to determine what services and care are needed.
Under the Program, palliative care services and support can be provided in the home, at Lions Gate Hospital, in Residential Care, or at the North Shore Hospice, depending on the needs and wishes of the patients.
To find out more about hospice and palliative care options available to you, talk to your family doctor about a referral to the Program.
